Cleveland, Ohio clinic performs US’s first face transplant

Thursday, December 18, 2008

A team of eight transplant surgeons in Cleveland Clinic in Ohio, USA, led by reconstructive surgeon Dr. Maria Siemionow, age 58, have successfully performed the first almost total face transplant in the US, and the fourth globally, on a woman so horribly disfigured due to trauma, that cost her an eye. Two weeks ago Dr. Siemionow, in a 23-hour marathon surgery, replaced 80 percent of her face, by transplanting or grafting bone, nerve, blood vessels, muscles and skin harvested from a female donor’s cadaver.

The Clinic surgeons, in Wednesday’s news conference, described the details of the transplant but upon request, the team did not publish her name, age and cause of injury nor the donor’s identity. The patient’s family desired the reason for her transplant to remain confidential. The Los Angeles Times reported that the patient “had no upper jaw, nose, cheeks or lower eyelids and was unable to eat, talk, smile, smell or breathe on her own.” The clinic’s dermatology and plastic surgery chair, Francis Papay, described the nine hours phase of the procedure: “We transferred the skin, all the facial muscles in the upper face and mid-face, the upper lip, all of the nose, most of the sinuses around the nose, the upper jaw including the teeth, the facial nerve.” Thereafter, another team spent three hours sewing the woman’s blood vessels to that of the donor’s face to restore blood circulation, making the graft a success.

The New York Times reported that “three partial face transplants have been performed since 2005, two in France and one in China, all using facial tissue from a dead donor with permission from their families.” “Only the forehead, upper eyelids, lower lip, lower teeth and jaw are hers, the rest of her face comes from a cadaver; she could not eat on her own or breathe without a hole in her windpipe. About 77 square inches of tissue were transplanted from the donor,” it further described the details of the medical marvel. The patient, however, must take lifetime immunosuppressive drugs, also called antirejection drugs, which do not guarantee success. The transplant team said that in case of failure, it would replace the part with a skin graft taken from her own body.

Dr. Bohdan Pomahac, a Brigham and Women’s Hospital surgeon praised the recent medical development. “There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Leading bioethicist Arthur Caplan of the University of Pennsylvania withheld judgment on the Cleveland transplant amid grave concerns on the post-operation results. “The biggest ethical problem is dealing with failure — if your face rejects. It would be a living hell. If your face is falling off and you can’t eat and you can’t breathe and you’re suffering in a terrible manner that can’t be reversed, you need to put on the table assistance in dying. There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Dr Alex Clarke, of the Royal Free Hospital had praised the Clinic for its contribution to medicine. “It is a real step forward for people who have severe disfigurement and this operation has been done by a team who have really prepared and worked towards this for a number of years. These transplants have proven that the technical difficulties can be overcome and psychologically the patients are doing well. They have all have reacted positively and have begun to do things they were not able to before. All the things people thought were barriers to this kind of operations have been overcome,” she said.

The first partial face transplant surgery on a living human was performed on Isabelle Dinoire on November 27 2005, when she was 38, by Professor Bernard Devauchelle, assisted by Professor Jean-Michel Dubernard in Amiens, France. Her Labrador dog mauled her in May 2005. A triangle of face tissue including the nose and mouth was taken from a brain-dead female donor and grafted onto the patient. Scientists elsewhere have performed scalp and ear transplants. However, the claim is the first for a mouth and nose transplant. Experts say the mouth and nose are the most difficult parts of the face to transplant.

In 2004, the same Cleveland Clinic, became the first institution to approve this surgery and test it on cadavers. In October 2006, surgeon Peter Butler at London‘s Royal Free Hospital in the UK was given permission by the NHS ethics board to carry out a full face transplant. His team will select four adult patients (children cannot be selected due to concerns over consent), with operations being carried out at six month intervals. In March 2008, the treatment of 30-year-old neurofibromatosis victim Pascal Coler of France ended after having received what his doctors call the worlds first successful full face transplant.

Ethical concerns, psychological impact, problems relating to immunosuppression and consequences of technical failure have prevented teams from performing face transplant operations in the past, even though it has been technically possible to carry out such procedures for years.

Mr Iain Hutchison, of Barts and the London Hospital, warned of several problems with face transplants, such as blood vessels in the donated tissue clotting and immunosuppressants failing or increasing the patient’s risk of cancer. He also pointed out ethical issues with the fact that the procedure requires a “beating heart donor”. The transplant is carried out while the donor is brain dead, but still alive by use of a ventilator.

According to Stephen Wigmore, chair of British Transplantation Society’s ethics committee, it is unknown to what extent facial expressions will function in the long term. He said that it is not certain whether a patient could be left worse off in the case of a face transplant failing.

Mr Michael Earley, a member of the Royal College of Surgeon‘s facial transplantation working party, commented that if successful, the transplant would be “a major breakthrough in facial reconstruction” and “a major step forward for the facially disfigured.”

In Wednesday’s conference, Siemionow said “we know that there are so many patients there in their homes where they are hiding from society because they are afraid to walk to the grocery stores, they are afraid to go the the street.” “Our patient was called names and was humiliated. We very much hope that for this very special group of patients there is a hope that someday they will be able to go comfortably from their houses and enjoy the things we take for granted,” she added.

In response to the medical breakthrough, a British medical group led by Royal Free Hospital’s lead surgeon Dr Peter Butler, said they will finish the world’s first full face transplant within a year. “We hope to make an announcement about a full-face operation in the next 12 months. This latest operation shows how facial transplantation can help a particular group of the most severely facially injured people. These are people who would otherwise live a terrible twilight life, shut away from public gaze,” he said.

Original Story
 • “Hotel development proposal could displace Buffalo, NY business owners” — Wikinews, February 17, 2006

In February of 2006, the Savarino Services Construction Corp. proposed the construction of a seven million dollar hotel on Elmwood and Forest Avenues in Buffalo, New York. In order for the hotel to be built, at least five properties containing businesses and residents would have to be destroyed. It was not certain whether the properties were owned by Savarino or by the landlord Hans Mobius. The hotel was designed by Karl Frizlen of the Frizlen Group, and is planned to be a franchise of the Wyndham Hotels group.

Elmwood Avenue is known by the community as a popular shopping center, and Nancy Pollina of Don Apparel (who is “utterly against” the construction) claims it’s the only reason why students from Buffalo State College leave campus. Additionally, Michael Faust of Mondo Video said he did not want to “get kicked out of here [his video store property].”

In 1995, a Walgreens was proposed to be built on the same land, but Walgreens later withdrew its request for a variance because of pressure from the community. More recently, Pano Georgiadis tried to get the rights to demolish the Atwater House next to his restaurant on Elmwood Avenue, but was denied a permit due to the property’s historical value. He has since been an opponent to the hotel construction.

In the process of debating the hotel, it was thought that a hotel had previously existed on the proposed site, however; research done at the Buffalo and Erie County Historical Society had shown that no hotel had previously existed on the site.

Nokia files lawsuit against Apple

Friday, October 30, 2009

Mobile phone and computer manufacturer Nokia is filing a lawsuit against Apple over patent infringements. The infringements include speech coding, wireless data, security and encryption technology Nokia claims is used in Apple iPhones.

According to a Nokia press release, 40 other companies are paying royalties to them in return for the use of their patented technology. Therefore, Nokia wants Apple to also pay for its usage.

“The basic principle in the mobile industry is that those companies who contribute in technology development to establish standards create intellectual property, which others then need to compensate for,” Ilkka Rahnasto, Nokia’s Vice President of Legal & Intellectual Property, said in the press release. “Apple is also expected to follow this principle. By refusing to agree appropriate terms for Nokia’s intellectual property, Apple is attempting to get a free ride on the back of Nokia’s innovation.”

One industry analyst told PC World that that he believes Nokia is seeking 1–2% royalties per device. Based on the number of iPhones sold since its 2007 release date, that figure would amount to approximately 400 million USD.

Ben Wood, research director at CCS Insight, said, “It is almost inconceivable that someone can produce a mobile phone without using Nokia patented technologies,” showing how widespread Nokia’s mobile technology is.

In Apple’s SEC 10-K, Apple responded to Nokia claims. “The complaint alleges that these patents are essential to one or more of the GSM, UMTS and 802.11 wireless communications standards, and that the Company [Apple] has the right to license these patents from plaintiff [Nokia] on fair, reasonable, and non-discriminatory terms and conditions. Plaintiff seeks unspecified FRAND compensation and other relief. The Company’s response to the complaint is not yet due. The Company intends to defend the case vigorously,” said Apple.

The Nokia v. Apple lawsuit is set to take place in a Delaware District Court.

Preventive Dentistry Helps Make Your Teeth Last A Lifetime

That alone is enough to ensure that your teeth will outlive you, right? Wrong. You see, while the enamel is tough, it’s also subject to wear and tear. Preventive dentistry is the special branch of dentistry that seeks to prevent certain conditions that may hinder the enamel from having a longer lifespan.

When the enamel is damaged, the softer and more vulnerable part of the tooth is exposed. This is actually the beginning of the end for the enamel. Once this protective coating is broken, nothing can undo it. If not treated early, tooth decay and gum disease will set in, leading to tooth loss. You can’t simply eat the right kinds of foods or take medicine to restore the enamel back to health. You can, however, take necessary precautions to ensure that your enamel is in tip-top condition; thus, preventing breakage. This is the core of preventive dentistry.

Preventive dentistry looks into the factors that lead to enamel loss and tries to prevent them. The best indication of enamel loss or damage is tooth sensitivity. When you experience tingling pain when you consume hot or cold foods and drinks, that’s one sure sign that your enamel has been compromised.

The most dangerous enemy of the enamel is erosion. Understanding what leads to tooth erosion is the key to preventing enamel damage, and this is the main job of preventive dentistry. One cause enamel erosion is acidic drinks. Sometimes, nothing quenches thirst as effectively as cold soft drinks and citrus fruit juices. The problem is, the acid in these drinks eats away at the enamel, making it susceptible to erosion.

Another reason for enamel erosion is the improper use of teeth bleaching agents. Be extra wary of those whitening strips which contain hydrogen peroxide or carbamide peroxide in large doses. If you want to whiten your teeth, the best thing for you to do is to let your dentist do it. A dentist especially skilled at preventive dentistry will know how to remove the discolouration in your teeth without putting the sensitive enamel at risk.

Preventive dentistry ensures that your teeth are free from chips and fractures which may expose the delicate interior of the tooth to bacteria. Corrective measures like fillings and dental veneers may be implemented to prevent tooth decay.

Most importantly, preventive dentistry provides excellent oral care for patients. Regular dental checkups and treatments combined with good dental hygiene at home help guarantee healthy teeth and gums and prevent costly restorations in the future.

HIV-positive man receives 35 years for spitting on Dallas police officer

Sunday, May 18, 2008

An HIV-positive man was sentenced to 35 years in prison Wednesday, one day after being convicted of harassment of a public servant for spitting into the eye and open mouth of a Dallas, Texas police officer in May 2006. The United States Centers for Disease Control and Prevention says that no one has ever contracted HIV from saliva, and a gay-rights and AIDS advocacy group called the sentence excessive.

A Dallas County jury concluded that Willie Campbell’s act of spitting on policeman Dan Waller in 2006 constituted the use of his saliva as a deadly weapon. The incident occurred while Campbell, 42, was resisting arrest while being taken into custody for public intoxication.

“He turns and spits. He hits me in the eye and mouth. Then he told me he has AIDS. I immediately began looking for something to flush my eyes with,” said Waller to The Dallas Morning News.

Officer Waller responded after a bystander reported seeing an unconscious male lying outside a building. Dallas County prosecutors stated that Campbell attempted to fight paramedics and kicked the police officer who arrested him for public intoxication.

It’s been 25 years since the virus was identified, but there are still lots of fears.

Prosecutors said that Campbell yelled that he was innocent during the trial, and claimed a police officer was lying. Campbell’s lawyer Russell Heinrichs said that because he had a history of convictions including similarly attacking two other police officers, biting inmates, and other offenses, he was indicted under a habitual offender statute. The statute increased his minimum sentence to 25 years in prison. Because the jury ruled that Campbell’s saliva was used as a deadly weapon, he will not be eligible for parole until completing at least half his sentence.

If you look at the facts of this case, it was clear that the defendant intended to cause serious bodily injury.

The organization Lambda Legal (Lambda Legal Defense and Education Fund), which advocates for individuals living with HIV, says that saliva should not be considered a deadly weapon. Bebe Anderson, the HIV projects director at Lambda Legal, spoke with The Dallas Morning News about the sentence. “It’s been 25 years since the virus was identified, but there are still lots of fears,” said Anderson.

The Dallas County prosecutor who handled the trial, Jenni Morse, said that the deadly weapon finding was justified. “No matter how minuscule, there is some risk. That means there is the possibility of causing serious bodily injury or death,” said Morse. Dallas County District Attorney Craig Watkins stated: “If you look at the facts of this case, it was clear that the defendant intended to cause serious bodily injury.”

Contact with saliva, tears, or sweat has never been shown to result in transmission of HIV.

A page at the CDC’s website, HIV and Its Transmission, states: “HIV has been found in saliva and tears in very low quantities from some AIDS patients.” The subsection “Saliva, Tears, and Sweat” concludes that: “Contact with saliva, tears, or sweat has never been shown to result in transmission of HIV.” On Friday the Dallas County Health Department released a statement explaining that HIV is most commonly spread through sexual contact, sharing needles, or transfusion from an infected blood product.

US Secretary Rice responds to European enquiries on alleged CIA prisons

Wednesday, December 7, 2005

The United States Secretary of State Condoleezza Rice has begun to address concerns raised by the EU, the Council of Europe, and several member countries about the CIA’s detention practices upon her arrival in Germany for a European tour that began Tuesday.

“As a matter of US policy, the United States’ obligations under the U.N. Convention Against Torture, which prohibits, of course, cruel and inhumane and degrading treatment, those obligations extend to US personnel wherever they are, whether they are in the United States or outside the United States,” said Rice, speaking from the Ukrainian capital of Kiev on Wednesday.

Media reports and Human Rights groups have alleged that the CIA transported renditioned prisoners through European countries, which could violate European laws and the sovereignty of countries involved. Secretary Rice claimed that the United States has respected the sovereignty of other countries, and that it has not transported detainees from one country to another for the purpose of interrogation using torture, and has not transported anyone to a country when we believe he will be tortured.

“We consider the captured members of Al-Qaeda and its affiliates to be unlawful combatants who may be held, in accordance with the law of war, to keep them from killing innocents. We must treat them in accordance with our laws, which reflect the values of the American people. We must question them to gather potentially significant, life-saving, intelligence. We must bring terrorists to justice wherever possible,” Rice told reporters before she left from Andrews Air Force base on Monday.

Rice said that European nations should realize that interrogations of terrorist suspects have produced information that has saved European lives. However, Secretary Rice provided no specific cases.

“Secretary Rice made extra-legal rendition sound like just another form of extradition. In fact, it’s a form of kidnapping and ‘disappearing’ someone entirely outside the law,” said Tom Malinowski, a Human Rights Watch official in Washington.

The CIA practice known as “extraordinary rendition” is used to interrogate terrorist suspects outside the U.S., where they are not subject to American legal protection.

“Kidnapping a foreign national for the purpose of detaining and interrogating him outside the law is contrary to American values,” said Anthony D. Romero, executive director of the American Civil Liberties Union (ACLU) on the Khalid El-Masri case. “Our government has acted as if it is above the law. We go to court today to reaffirm that the rule of law is central to our identity as a nation.”

The ACLU feels the government has to be held to account over “extraordinary rendition”.

Making Kitchen Clean Using Natural Cleaning Products

By Markus Skupeika

Kitchen is the area where foods are prepared. So, your kitchen should always be clean and sanitized. It should be the cleanest part of your house in order to ensure that you are eating good and safe food. But unfortunately, it is sometimes neglected because other people tend to spend more time in cleaning their rooms, bathrooms, comfort rooms and other areas of the house. This habit can greatly endanger your family’s health so better spend more time in cleaning your kitchen.

Clean your whole kitchen with the natural cleaning products to avoid chemical ingredients that you can get from chemical based cleaning solutions. You can buy these green cleaning products or you can produce them yourself. Either way is a safer than going for conventional cleaning products.

Here are good and easy tips on how you can make your kitchen the most sanitized room in your house:

– Play your favorite song or any other song before you start to lighten up your mood in cleaning your kitchen.

– Start by picking up pieces of dirt or any misplaced items or things.

– Place or put the misplaced things into their proper places.

– If there are used dishes, wash them properly and carefully to avoid damage or accident.

Steps in cleaning dishes properly:

– Start cleaning the drinking glasses and mugs because they are the items of least food stains and grease. They are fragile or easily broken so better be careful in cleaning them.

– Next to them are the plates, bowls and saucers or what they call china wares. Clean them carefully because they too are fragile or are easily broken.

– And the last are the pots and pans. Consider to clean them always at the last part because they are the dirtiest ones. Since they are the dirtiest, soak them for a few minutes to remove or to loosen the food stains.

– In general, wash your kitchen utensils with warm water.

– You can use hot water to remove grease easily.

– Scrub those dishes carefully and thoroughly. Choose to use dishwashing soap or solution made out natural cleaning products. This will make your entire cleaning process safe from chemical ingredients.

Here are the natural ingredients that you can use in cleaning your kitchen:

– White distilled vinegar

– Baking soda

– Washing soda

– Baking soda

– Lemon juice

– Lemon and lime

– Hot water

– Olive oil

– Herbs

– Salt

– Tooth paste (white or plain)

– Borax

– Cream of tartar

Here’s how to clean your kitchen using green cleaning products:

– Usually, pans turn black after a long time of use so clean them with white vinegar and baking soda with the use of a sponge.

– Do not forget to dry metal equipments. Because they are metallic, they are prone to rusting.

– Wipe your range surface after cooking. Apply lemon juice on a small rug or piece of old cloth then simply wipe the surface that you are cleaning.

– Clean kitchen floor every day. Add 1 cup of white distilled vinegar to every gallon of water in a pail. Dip or soak your mop into the pail then simply wipe your floor.

– Don’t forget to dry the kitchen floor after every wipe; it may cause accidents like slipping.
