Cleveland, Ohio clinic performs US’s first face transplant

Thursday, December 18, 2008

A team of eight transplant surgeons in Cleveland Clinic in Ohio, USA, led by reconstructive surgeon Dr. Maria Siemionow, age 58, have successfully performed the first almost total face transplant in the US, and the fourth globally, on a woman so horribly disfigured due to trauma, that cost her an eye. Two weeks ago Dr. Siemionow, in a 23-hour marathon surgery, replaced 80 percent of her face, by transplanting or grafting bone, nerve, blood vessels, muscles and skin harvested from a female donor’s cadaver.

The Clinic surgeons, in Wednesday’s news conference, described the details of the transplant but upon request, the team did not publish her name, age and cause of injury nor the donor’s identity. The patient’s family desired the reason for her transplant to remain confidential. The Los Angeles Times reported that the patient “had no upper jaw, nose, cheeks or lower eyelids and was unable to eat, talk, smile, smell or breathe on her own.” The clinic’s dermatology and plastic surgery chair, Francis Papay, described the nine hours phase of the procedure: “We transferred the skin, all the facial muscles in the upper face and mid-face, the upper lip, all of the nose, most of the sinuses around the nose, the upper jaw including the teeth, the facial nerve.” Thereafter, another team spent three hours sewing the woman’s blood vessels to that of the donor’s face to restore blood circulation, making the graft a success.

The New York Times reported that “three partial face transplants have been performed since 2005, two in France and one in China, all using facial tissue from a dead donor with permission from their families.” “Only the forehead, upper eyelids, lower lip, lower teeth and jaw are hers, the rest of her face comes from a cadaver; she could not eat on her own or breathe without a hole in her windpipe. About 77 square inches of tissue were transplanted from the donor,” it further described the details of the medical marvel. The patient, however, must take lifetime immunosuppressive drugs, also called antirejection drugs, which do not guarantee success. The transplant team said that in case of failure, it would replace the part with a skin graft taken from her own body.

Dr. Bohdan Pomahac, a Brigham and Women’s Hospital surgeon praised the recent medical development. “There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Leading bioethicist Arthur Caplan of the University of Pennsylvania withheld judgment on the Cleveland transplant amid grave concerns on the post-operation results. “The biggest ethical problem is dealing with failure — if your face rejects. It would be a living hell. If your face is falling off and you can’t eat and you can’t breathe and you’re suffering in a terrible manner that can’t be reversed, you need to put on the table assistance in dying. There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Dr Alex Clarke, of the Royal Free Hospital had praised the Clinic for its contribution to medicine. “It is a real step forward for people who have severe disfigurement and this operation has been done by a team who have really prepared and worked towards this for a number of years. These transplants have proven that the technical difficulties can be overcome and psychologically the patients are doing well. They have all have reacted positively and have begun to do things they were not able to before. All the things people thought were barriers to this kind of operations have been overcome,” she said.

The first partial face transplant surgery on a living human was performed on Isabelle Dinoire on November 27 2005, when she was 38, by Professor Bernard Devauchelle, assisted by Professor Jean-Michel Dubernard in Amiens, France. Her Labrador dog mauled her in May 2005. A triangle of face tissue including the nose and mouth was taken from a brain-dead female donor and grafted onto the patient. Scientists elsewhere have performed scalp and ear transplants. However, the claim is the first for a mouth and nose transplant. Experts say the mouth and nose are the most difficult parts of the face to transplant.

In 2004, the same Cleveland Clinic, became the first institution to approve this surgery and test it on cadavers. In October 2006, surgeon Peter Butler at London‘s Royal Free Hospital in the UK was given permission by the NHS ethics board to carry out a full face transplant. His team will select four adult patients (children cannot be selected due to concerns over consent), with operations being carried out at six month intervals. In March 2008, the treatment of 30-year-old neurofibromatosis victim Pascal Coler of France ended after having received what his doctors call the worlds first successful full face transplant.

Ethical concerns, psychological impact, problems relating to immunosuppression and consequences of technical failure have prevented teams from performing face transplant operations in the past, even though it has been technically possible to carry out such procedures for years.

Mr Iain Hutchison, of Barts and the London Hospital, warned of several problems with face transplants, such as blood vessels in the donated tissue clotting and immunosuppressants failing or increasing the patient’s risk of cancer. He also pointed out ethical issues with the fact that the procedure requires a “beating heart donor”. The transplant is carried out while the donor is brain dead, but still alive by use of a ventilator.

According to Stephen Wigmore, chair of British Transplantation Society’s ethics committee, it is unknown to what extent facial expressions will function in the long term. He said that it is not certain whether a patient could be left worse off in the case of a face transplant failing.

Mr Michael Earley, a member of the Royal College of Surgeon‘s facial transplantation working party, commented that if successful, the transplant would be “a major breakthrough in facial reconstruction” and “a major step forward for the facially disfigured.”

In Wednesday’s conference, Siemionow said “we know that there are so many patients there in their homes where they are hiding from society because they are afraid to walk to the grocery stores, they are afraid to go the the street.” “Our patient was called names and was humiliated. We very much hope that for this very special group of patients there is a hope that someday they will be able to go comfortably from their houses and enjoy the things we take for granted,” she added.

In response to the medical breakthrough, a British medical group led by Royal Free Hospital’s lead surgeon Dr Peter Butler, said they will finish the world’s first full face transplant within a year. “We hope to make an announcement about a full-face operation in the next 12 months. This latest operation shows how facial transplantation can help a particular group of the most severely facially injured people. These are people who would otherwise live a terrible twilight life, shut away from public gaze,” he said.

Aesthetic Skin Care Tips: Simple Secrets To Minimizing Pore Size}

Submitted by: Dr Tyng Tan

One of the factors that contribute to the overall appearance of your face are your pore size. The smaller the pores, better. When they increase in diameter which can be caused by constant sun exposure, poor skin care, smoking, improper diet, acne, skin dehydration, or hormonal changes, it is when the problem starts.

Aside from the unattractiveness that an enlarged pore size does on your face, it introduces dirt, grime and bacteria into your skin. This can lead to an inflammatory response which can cause other skin condition, with acne as the most common among these as the pores become clogged.

So better minimize those pores through these simple tips which you can do on your own!

One of the simple remedy for enlarged pores which you can do at home is through steam therapy. You pour boiling water into a basin and at a safe distance, about 14 inches apart; you place your face above the basin. Place a towel over yourself and the basin to contain the steam.

The steam opens up your pores and stimulates your skin to produce sweat. As the pores open the sweat helps remove the dirt and oil that is clogging it. Do this for about 15 minutes and then wipe of the sweat with a clean towel after you are done.

You then follow this step by washing your face with fresh lemon juice. The acidic ph of lemon juice can help tighten your face and minimize your pores. In addition to that, lemon also has antioxidant properties that will help cleanse the skin and the pores better.

If you continue to do this for the next two weeks, you will see your face become tighter, smoother and glowing.

Rubbing ice on your face can also tighten your pores. Do this for 5-10 minutes daily and over time you will notice your skin being tightened better.

Another way is to mix yourself your own facial mask that will help minimize your pore size. By mixing salt and buttermilk and make it into a paste, you apply this on your face particularly on that space where the enlarged pores are concentrated. You leave it on for 5-7 minutes and then wash it off.

Other kitchen finds that you can use as face masks include mashed cucumber and mashed raw potato. You can alternate the two and leave them on your face for 15 minutes. They can help clear your skin, lighten it and effectively minimize an enlarged pore size.

You can also use the mixture of 2 tablespoons oatmeal and half a tablespoon of buttermilk as a facial mask. You leave this on your face for 20 minutes and then rinse it off, then you follow this with ice cube rub for better pore minimizing effect.

The juices of pineapple, peaches, papaya and tomatoes can be rubbed all over the face because these also have skin tightening properties. After you leave it on for a few minutes, you then rinse it with warm water.

James Brown dies of pneumonia

Monday, December 25, 2006

James Brown, often referred to as the Godfather of Soul, died in Atlanta due to congestive heart failure, combined with pneumonia. His death at age 73 was announced by his agent. After his dentist noticed something unusual with him, Brown was told to visit a doctor immediately. He was taken into the hospital yesterday for treatment of his pneumonia until his death at around 1:45 AM (6:45 AM GMT). It is not known whether he received a pneumonia vaccination, as recommended for people of his age.

He was born in 1933 and grew up in poverty until he formed James Brown & The Famous Flames. His influence on 20th century music, from funk to hip hop was profound.

Before he died, he scheduled a New Year’s Eve concert series in New Jersey and New York that would help kick off a 2007 tour.

Hundreds of victims still unidentified three months after Typhoon Frank

Tuesday, September 30, 2008

Typhoon Frank stormed across the Philippine Sea June 21, 2008 capsizing numerous boats, killing hundreds and leaving an estimated 1,330 missing in total including 820 passengers and crew from the MV Princess of the Stars ferry.

Since then Disaster Victim Identification (DVI) teams in Cebu City, Philippines made up of experts from the Philippines National Bureau of Investigation (NBI), the International Commission on Missing Persons (ICMP). and INTERPOL have painstakingly matched DNA from recovered bodies to that of blood samples donated by relatives of those still missing.

“The 23,000-ton vessel was also carrying tens of thousands of pounds of commercial insecticide and many bodies still remain trapped inside the wreckage” according to a recent press release from the ICMP.

“With fingerprint identification out of the question for most of the victims, and dental records not available, this leaves only DNA as a means of identification, with samples from recovered victims matched to those provided by the missing passengers’ relatives.”

An NBI-DVI report explains “DNA matches are made by comparing profiles from victims to a database of profiles from their relatives. A minimum of two close relatives [are] sought for each individual.”

NBI-DVI has so far collected 1,916 blood samples from relatives of missing victims representing 809 missing persons. These samples are then matched against the 318 DNA samples from recovered bodies received by the ICMP.

“Almost three months have passed since our country was struck by one of the worst typhoons in memory, with ‘Frank’ causing the sinking of the Princess of the Stars with hundreds of passengers and crew on board,” said NBI Director Nestor M. Mantaring in the press release.

“The NBI, through its DVI team, was tasked [with identifying] the victims of this tragedy. With help from INTERPOL and the ICMP, we have reached the 100th DNA-matched result which led to the identification and release of the bodies to their respective families.

“We are committed to doing this until the last possible victim is identified and returned to their family,” he concluded.

Looted, possibly contaminated body parts transplanted into USA, Canadian patients

Monday, March 20, 2006

Fears of contaminated bone and skin grafts are being felt by unsuspecting patients following the revelation that funeral homes may have been looting corpses.

Janet Evans of Marion Ohio was told by her surgeon, “The bone grafts you got might have been contaminated”. She reacted with shock, “I was flabbergasted because I didn’t even know what he was talking about. I didn’t know I got a bone graft until I got this call. I just thought they put in screws and rods.”

The body of Alistair Cooke, the former host of “Masterpiece Theatre,” was supposedly looted along with more than 1,000 others, according to two law enforcement officials close to the case. The tissue taken was typically skin, bone and tendon, which was then sold for use in procedures such as dental implants and hip replacements. According to authorities, millions of dollars were made by selling the body parts to companies for use in operations done at hospitals and clinics in the United States and Canada.

A New Jersey company, Biomedical Tissue Services, has reportedly been taking body parts from funeral homes across Brooklyn, New York. According to ABC News, they set up rooms like a “surgical suite.” After they took the bones, they replaced them with PVC pipe. This was purportedly done by stealth, without approval of the deceased person or the next of kin. 1,077 bodies were involved, say prosecuters.

Investagators say a former dentist, Michael Mastromarino, is behind the operation. Biomedical was considered one of the “hottest procurement companies in the country,” raking in close to $5 million. Eventually, people became worried: “Can the donors be trusted?” A tissue processing company called LifeCell answered no, and issued a recall on all their tissue.

Cooke’s daughter, Susan Cooke Kittredge, said, “To know his bones were sold was one thing, but to see him standing truncated before me is another entirely.” Now thousands of people around the country are receiving letters warning that they should be tested for infectious diseases like HIV or hepatitis. On February 23, the Brooklyn District Attorney indicted Mastromarino and three others. They are charged with 122 felony counts, including forgery and bodysnatching.

Improving Life In Delhi}

Improving Life in Delhi

by

Deepika bansal

The city of Delhi is one of the busiest places in India being that national capital. It hardly has any space left to accommodate more outsiders as it is fully choked with the ever growing population. The government of Delhi is trying to make it a better place to live since it is now so full that it seems choked.

The most glorious development in Delhi is that of Metro Train, it has become one of the most important milestones in the history of the city. In just few years, commutability has certainly become lot easier and comfortable for people. Till sometime back, it was among the major issues Delhi was dealing with but now it is all set to connect all the parts of city with each other. Everyone is exuberant with the outcome of this project while the State government is winning applauses and accolades for the same. Metro has already connected North, East and West Delhi and would bring South and Central Delhi closer soon. In fact, NCR regions of Gurgaon and Noida are going to get nearer to the city adding all the more to residents happiness.

Removal of illegal commercial establishments

Delhi is occupied with a lot of illegal encroachments due to ceaseless activities taking place. Keeping a constant vigil on developments of such illegal commercial properties like shops, banquet halls, towers, etc. sometimes gets out of hands of the government. The eruption of shops and banquets in residential areas cause a lot of disturbance and discomfort to the residents, becoming an issue of concern.

But, to address the problem, a recent order passed by Supreme Court demands closing of all the illegal shops and the banquet halls in residential spaces. The presence of shops in every 20 metres may sometimes cause comfort but is mostly a reason of trouble. Wide spaces cannot be found and even footpaths meant for pedestrians seem to have disappeared. There are more than 5 lakh illegal shops facing the heat of sealing operation in spite of the continuous protests and demonstrations. Supreme Court orders to the MCD on sealing of all the commercial establishments in residential areas would apply to banquet halls as well. They result in unnecessary traffic congestion and noise pollution for home dwellers and thus, the sealing drive is on to provide them relief from the chaos.

Not only this, the sealing sword is hanging over heads of illegal mobile tower operators as well. Most of these towers are in residential areas and are expected to move out to commercial areas. These towers are said to pose health risks by radiating harmful waves and so, are asked to find another place. Relocating the mobile towers is a problematic task that is bound to create major dislocation of service and cause inconvenience to the users. But, joint working of authorities and service providers to resolve the issue can make it a success.

Proposed Developments

Other issues addresses by government bodies include availability of car parking sites and of residential spaces.

The development of car parking sites in residential areas of Delhi property has been proposed by MCD (Municipal Corporation of Delhi) to address the car parking issues. This would make space for about one lakh cars on free basis and is expected to complete within a year or so. This news has brought smiles on many faces as acute shortage of parking space was another complication for the residents.

The government is working on construction of around 6, 000 houses for economically Weaker Section (EWS) of the society. In next one year, the housing for people from financially weak background would be ready for use in areas like Samaspur Khalsa in Najafgarh, Bhorgarh near Narela, etc. EIFL promises use of cost effective technology and quality housing for these people.

Living in Delhi is all set to become a completely new experience with huge developments taking place.
